Released from a prison mental ward after 40 years, Coffin Joe meets with his old and loyal henchman Bruno who takes him to his new shelter in a shanty neighborhood of São Paulo. The hideout is populated by a cult of psychotics obsessed by Coffin Joe's ideas and that were waiting for his arrival. Coffin Joe immediately orders the members of the cult to look for beautiful women so he can re-start his bloody quest for "the continuation of the blood", where he finds the perfect woman who will bear him a son.
